Documents Required to Change the Guardian on a Minor Account
The following table outlines the documents necessary to update or change the guardian associated with a minor's demat account:

Application
Forms - Equity application form (PDF) - KYC form for the new guardian (PDF) - CKYC form for the minor (PDF) - CKYC form for the new guardian (PDF) - Account modification form (PDF) - Refer to the signature guidelines and sample copy.
PAN - Self-attested copy of the new guardian’s PAN - Copy of the minor’s PAN attested by the new guardian
Proof of Date of
Birth (DOB) Any one of the following: - Birth certificate - School leaving certificate - Passport - Mark sheet issued by a higher secondary board
Photographs - Passport-size photograph of the minor - Passport-size photograph of the new guardian
Bank Proof Any one of the following in the minor's name: - Cancelled cheque - Latest bank statement or passbook
Legal Guardian
Letter Required if the new guardian is not a biological parent. Submit a copy of the legal guardianship letter.
For both the minor (attested by the new guardian) and the new guardian. Acceptable documents include: - Passport - Masked Aadhaar Steps to download masked Aadhar 1. Visit myaadhaar.uidai.gov.in. 2. Click on Login. 3. Enter the Aadhaar number, Captcha and click on Send OTP.
Document Type Requirements 4. Enter the OTP received on the Aadhaar registered mobile number and click on Login. 5. Click on Download Aadhaar. 6. Click on Do you want a masked Aadhaar? 7. Click on Download. 8. Click on the downloaded file and enter the first four letters of your name as per Aadhaar in CAPITAL letters and year of birth in YYYY format. Address proof - Driver's license (guardian only) - Voter ID (guardian only)
Death
Certificate If the current guardian is deceased, a notarized or gazette officer-attested copy of the death certificate must be submitted.
Court Order If the change in guardianship is based on a legal ruling, a notarized or gazette officer-attested copy of the court order is required.
